Summary of Duties: Member of multidisciplinary health care team, facilitates patient movement through continuum of care. Make decisions regarding utilization management & alternative treatment for cost effectiveness. Interfaces with internal, external departments. & agencies. Educational Requirements - RN license, BS or BSN is desired. Licensure/Certification Requirement - Registered Nurse currently licensed in State of California and BLS Certification. Experience- One-year recent experience in Utilization Case Management or Discharge Planning is mandatory. Current acute care experience required. Current knowledge of reimbursement regulations, levels of care.Special Skills or abilities - Communicate effectively, prepare oral and written reports, computer literate, can abstract data from medical records and enter in software system.
